Job summary

Elevance Health, Inc. is seeking a CFSP Managed Care Coordinator to operate primarily in the Pitt County, NC area. This field-based role requires travel to client sites and coordination with county welfare and health services to deliver integrated care for Medicaid-enrolled individuals.

The coordinator will assess needs, collaborate with a multidisciplinary team, and support care management across physical health, behavioral health, and related services. Residency in North Carolina is preferred.

Qualifications

• BA/BS degree in health-related field or equivalent.
• Experience with trauma-informed care and child welfare.
• Care coordination experience with multidisciplinary teams.

Responsibilities

• Provide integrated care management across physical and behavioral health.
• Coordinate with county Child Welfare workers to align health care with permanency goals.
• Assess member needs via telephonic or in-person interactions and coordinate services.
