Elevance Health seeks a CFSP Managed Care Coordinator in North Carolina, field-based in Pitt County (Greenville). The role focuses on integrated care management across physical health, behavioral health, I/DD, LTSS, and pharmacy, with emphasis on trauma-informed and collaborative care.

The position involves traveling to client sites, partnering with county child welfare workers, and coordinating with a multidisciplinary team to meet members' comprehensive care needs.

Formación

• Requires BA/BS degree and a minimum of 1 year of experience working directly with the target population or related community organizations.
• Two (2) years of experience working directly with individuals served by the child welfare system is preferred.
• Residence in North Carolina preferred.
• Colocation possibility may require reporting to an office 1-3 days per week.
• Travel to worksites and other locations as necessary.

Responsabilidades

• Provide integrated whole-person Care Management under the CFSP model across physical and behavioral health, I/DD, LTSS, and related services.
• Coordinate care across a multidisciplinary team including primary care and specialty providers.
• Conduct telephonic or face-to-face assessments to identify and manage member program needs.
• Oversee non-clinical needs of members with chronic illnesses or disabilities for cost-effective service use.
• Set short- and long-term goals with members, caregivers, and physicians.
• Identify members who would benefit from expanded services.
