Job summary

Elevance Health, Inc. is seeking a CFSP Managed Care Coordinator to oversee individual service plans within the NC CFSP program. This field-based role requires strong care coordination across physical, behavioral health, and LTSS services.

You will collaborate with county welfare workers, execute assessments, set goals with members and families, and ensure integrated care management for Medicaid beneficiaries. Travel to client sites is expected.

Qualifications

• Requires BA/BS degree with equivalent background or related experience.

Responsibilities

• Provide integrated whole-person care management under the CFSP model, coordinating across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet health-related needs.
• Collaborate with County Child Welfare Workers to align health care with permanency goals.
• Coordinate care with multidisciplinary teams including primary and behavioral health professionals and other stakeholders.
• Conduct assessments to identify, evaluate, and manage member program needs via telephonic or in-person encounters.
• Oversee non-clinical needs of members with chronic illnesses or disabilities for cost-effective service use.
• Set short- and long-term goals with members, families, and physicians.
• Identify members who may benefit from expanded services.
