Job summary

Elevance Health, Inc. in North Carolina seeks a CFSP Managed Care Coordinator to support Medicaid infants, children, youth, and families with integrated care management in a field-based role serving Columbus & Bladen counties.

The position requires a BA/BS and at least 1 year of related experience, with travel to client sites four to five days a week and occasional office attendance. Emphasis on trauma-informed, person-centered care and collaboration with child welfare staff.

Qualifications

• Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the specific program population or other related community-based organizations; or any combination of education and experience which would provide an equivalent background.
• Trauma-Informed Care understanding is preferred to support CFSP population.
• Strong ability to coordinate care across multiple health and social services.

Responsibilities

• Provide integrated whole-person Care Management under the CFSP Care Management model, including coordination across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet health-related needs.
• Collaborate with County Child Welfare Worker to align health care needs with permanency planning goals.
• Work with a multidisciplinary care team to coordinate care across providers and services.
• Conduct telephonic or face-to-face assessments to identify, evaluate, coordinate, and manage member program needs.
• Identify members who would benefit from expanded services and coordinate their cases with the team.
