Job summary

Elevance Health, Inc. is seeking a Field-based FSP Managed Care Coordinator in North Carolina’s Pitt County (Greenville) to support Medicaid CFSP members.

The role emphasizes hands-on engagement with patients and families, traveling to client sites and maintaining in-person interactions four to five days a week. The position requires a BA/BS and 1+ year of relevant experience, with travel to various sites and limited onsite office attendance.

Qualifications

• Requires BA/BS degree and at least 1 year of direct experience with the target program population or related community-based organizations.
• Must reside in North Carolina.
• Two years of experience with child welfare is preferred.
• Colocation may require reporting to an office 1–3 days per week; travel to worksites as needed.

Responsibilities

• Provide integrated care management across physical health, behavioral health, and related services.
• Coordinate with county child welfare workers to align health needs with permanency goals.
• Collaborate with a multidisciplinary care team to coordinate care and services.
• Conduct telephonic or in-person assessments to identify and manage member needs.
• Oversee non-clinical needs of members with chronic conditions to optimize resource use.
• Set short- and long-term goals with members and caregivers.
