Job summary

NeuLife Rehabilitation in Frankfort, KY seeks a Care Coordinator to manage Medicaid authorizations, medical appointments, and case coordination across residential and community services. You will serve as the central contact ensuring regulatory compliance and care continuity for all persons served.

The role involves coordinating with the RN Executive Director, supervising the ADT team, and maintaining accurate records and timely communications with guardians and providers.

Qualifications

• Experience in healthcare coordination or Medicaid authorization.
• Strong organizational and multitasking abilities.
• Excellent written and verbal communication skills.
• Ability to supervise and coordinate team activities.
• Knowledge of Medicaid waiver programs (ABI Waiver experience a plus).

Responsibilities

• Manage all State Medicaid Plan of Care (POC) and ABI Waiver authorizations, submissions, and tracking.
• Coordinate medical appointments and transportation.
• Serve as primary Point of Contact for all case managers and guardians.
• Oversee ADT Team operations and interdepartmental coordination.
• Coordinate with RN Executive Director on medication management and program support.
• Maintain accurate logs of appointments, follow-ups, and outcomes.
• Ensure records are organized, accurate, and accessible for audits.
