Qualifications

• Must possess an active nursing license and be eligible for CCM/ACM certification.
• Bachelor's degree in a health-related field or higher preferred.
• 2 years rehabilitation experience or equivalent preferred.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Work with interdisciplinary team to guide treatment plans.
• Participate in discharge planning and documentation.
• Monitor and ensure compliance with regulations and payor requirements.
