Job summary

Qualifications

• One-year experience in medical field or customer service preferred.
• Background check and fingerprinting required.
• Multilingual candidates encouraged (Spanish/Somali/Nepali/Haitian Creole).

Responsibilities

• Complete outreach calls to patients who miss visits and schedule care.
• Reach patients due for wellness visits or chronic care and arrange appointments.
• Use UnityPHM text tools to contact hard-to-reach patients.
• Support patients with insurance applications and connect to CHWs when needed.
• Collaborate within a team to reduce barriers to care.
