Job summary

Heart of Ohio Family Health Centers is seeking a Patient Navigator to engage Medicaid patients and those missing visits. You will schedule care, assist with medications and referrals, and use UnityPHM to text patients.

The role requires strong customer service and teamwork within a diverse clinic environment. Experience in medical or customer service fields is preferred; multilingual candidates are encouraged to apply.

Qualifications

• One-year experience in medical field or customer service preferred.
• Education in community health worker, medical assistant, public health, or related field preferred.
• Multilingual candidates encouraged (Spanish/Somali/Nepali/Haitian Creole).

Responsibilities

• Complete outreach calls to patients missing visits and schedule care.
• Schedule patient visits and assist with routine medical care.
• Contact patients due for medications or referrals and arrange follow-ups.
• Text patients via UnityPHM when reachable by phone.
• Address patient concerns and aid insurance applications or CHW referrals.
• Collaborate with staff to reduce barriers and support diverse patients.
