Job summary

The Long Island State Veterans Home, located on the campus of Stony Brook University, seeks a Medicaid Coordinator to assist with admissions and ensure continued Medicaid coverage for eligible residents.

Responsibilities include interviewing applicants, preparing Medicaid applications, recertifications, and conversions, while maintaining communications with families and DSS offices. Strong writing, communication and Excel skills are essential.

Qualifications

• Associate's Degree or higher plus a minimum of three (3) years preparing Chronic Care Medicaid applications and recertifications.
• High School Diploma plus five (5) years preparing Chronic Care Medicaid applications and recertifications.
• Experience submitting cases with Suffolk County, Nassau County and NYC HRA; proficient in Word and Excel; strong communication and customer service skills.

Responsibilities

• Interview applicants and analyze financial documents to establish Chronic Care Medicaid eligibility, identifying issues within a five-year look-back period.
• Review Medicaid Checklist and counsel applicants on required documents and deadlines.
• Prepare Medicaid applications and conduct face-to-face interviews to satisfy NYS DSS regulations; submit and maintain communications with DSS.
• Process annual Medicaid recertification for residents and handle conversions from community to institutional coverage.
• Maintain communications with families, attorneys, geriatric care managers and county DSS; prepare Medicaid tracking reports for senior management.
• Maintain relationships with Suffolk County DSS, Nassau County DSS, NYC HRA and other county DSS offices; attend Medicaid training.
• Explain Medicaid Chronic Care eligibility and document requirements to applicants considering nursing home placement; keep current with Medicaid laws.
• Maintain a working relationship with the Business Office to expedite Medicaid budgets, NAMI, budget corrections and MLTC enrollments.
• Prepare reports identifying critical cases for review at scheduled meetings; ensure compliance with regulations and policies.
• Assist Admissions Director with eligibility issues; perform other tasks as assigned by CFO and Director of Admissions.
