Responsibilities

• Interview applicants and analyze financial documents to establish Chronic Care Medicaid eligibility.
• Review Medicaid Checklist and counsel applicants on required documents and deadlines.
• Prepare Medicaid applications and conduct face-to-face interviews per NYS DSS regulations; submit applications and maintain contact with DSS caseworkers.
• Process annual Medicaid recertifications and conversions for residents as applicable.
• Maintain communication with families, attorneys, geriatric care managers and county DSS offices; prepare Medicaid tracking reports.
