You will verify documentation, collaborate with DSS, and ensure coverage upon release, coordinating with the JI reentry portal and CorEMR for billing.

Qualifications

• High school diploma or equivalent is required.
• Experience in a correctional facility or healthcare environment preferred.
• CPR certification must be maintained; First Aid may be required by contract.
• Familiarity with Medi-Cal eligibility, public benefits enrollment, or healthcare coverage programs preferred.
• Strong attention to detail, organization, data entry, and communication skills essential.

Responsibilities

• Identify and screen incarcerated individuals for Medi-Cal eligibility.
• Support submissions of new applications, renewals, and reactivations for individuals in custody.
• Verify required documentation and collaborate with DSS to process pre-release applications to ensure coverage.
• Work with discharge planners to ensure the job aid code has been activated in the JI reentry portal.
• Enter CIN numbers into CorEMR for billing purposes.
