Amity Foundation seeks a Medi-CAL Coordinator to screen individuals for Medi-CAL eligibility, coordinate STOP placements, and link housing, substance use treatment, and community resources. You will work with Parole Offices, ECM staff, and Reentry teams to ensure timely and appropriate service enrollment in ARMS.

The role requires collaboration across medical, behavioral, and social services, strong communication, and knowledge of Medi-CAL, DMC, CalAIM systems.

Responsabilidades

• Screen eligibility for Medi-CAL and STOP referrals.
• Generate placement recommendations based on screening results.
• Coordinate across Medi-CAL, DMC, and CalAIM systems of care.
• Enter enrollment data into ARMS and ensure documentation is complete.
• Collaborate with Parole Offices and program staff for timely placements.
• Support outreach and engagement activities for eligible participants.
• Supervise/coordinate tasks with Reentry Assessment staff as needed.

Bachelor’s degree in social work, psychology, public health or related field
