Job summary

Brookdale Senior Living is seeking an experienced MDS Specialist RN for our skilled nursing facility in Brentwood, CA. You will oversee MDS assessments, care planning, and interoperability with clinical teams to ensure regulatory compliance and high-quality resident care.

We value leadership, a nursing degree, and at least two years of supervisory experience in a hospital, nursing care setting, or related health care facility. Strong knowledge of rehabilitative nursing practices is required.

Qualifications

• Min. 2 years of supervisory experience in a hospital, nursing care community, or related health care facility.
• Must possess a Nursing Degree from an accredited college or university.
• Must have current State RN license.

Responsibilities

• Coordinate development and completion of resident assessments (MDS).
• Lead care planning with interdisciplinary team and ensure timely MDS transmission.
• Ensure compliance with CA and state guidelines and regulatory requirements.
• Provide leadership in discharge planning and quality assurance activities.
