Job summary

Brookdale is seeking a Nursing Supervisor in Texas to oversee resident assessment and care planning within our senior living communities. This role ensures regulatory compliance, coordinates MDS timelines, and leads a multidisciplinary team to deliver quality resident care.

The position requires a valid RN license and at least two years of supervisory experience in a hospital or long-term care setting, plus six months of rehab nursing training.

Qualifications

• RN license required.
• Minimum 2 years of experience as a supervisor in a hospital, nursing care community, or related health care facility.
• Minimum 6 months training experience in rehabilitative and restorative nursing.

Responsibilities

• Coordinates the development and completion of the resident assessment (MDS) per regulations.
• Coordinates the written plan of care for residents with measurable objectives and timetables.
• Ensures timely generation and transmission of MDS and maintains care plan policies.
• Develops and maintains an ongoing QA program for resident assessment and care plans.
• Leads interdisciplinary teams and participates in training for assessment skills and MDS processes.
