Job summary

Brentwood Campus of Care is seeking a lead MDS Coordinator to oversee RAI and care management processes from admission to discharge, ensuring compliance with company policy and federal/state guidelines. The role reports to the Director of Nursing and VP of Clinical Reimbursement and coordinates with the clinical team campus-wide.

The position expects RN licensure in the state and values a strong background in MDS 3.0, care planning, and Medicaid/Medicare reimbursement knowledge.

Qualifications

• Graduate of an approved RN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process, OBRA regulations and Minimum Data Set.
• Experience with MDS 3.0 is required.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings and communicate resident care and assessment dates.
• Ensure compliance with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation in the medical record and maintain MDS status.
• Support staff in RAI processes, PPS Medicare/Medicaid, and clinical computer systems.
