Job summary

Brentwood Place Three seeks an experienced MDS Coordinator to oversee RAI and care management from admission to discharge, ensuring compliance with Federal, State, and Certification guidelines. The role reports to the Director of Nursing and involves coordinating information systems and education for the clinical team.

Responsibilities include scheduling MDS assessments, maintaining coding accuracy, and ensuring timely submission of documentation.

Qualifications

• Graduate of an approved RN program and licensed in the state.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding OBRA requirements and the Minimum Data Set.
• Experience with MDS 3.0.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Ensure decisions on MDS completeness align with regulatory criteria.
• Coordinate daily stand up meetings and communicate outcomes to staff and families.
• Maintain accurate MDS coding and documentation in medical records.
• Submit electronic documentation to state databases and other entities per policy.
• Train and assist staff on RAI, PPS, and clinical computer systems.
