Job summary

Millington Healthcare is seeking a dedicated and experienced MDS Director to join our team at our Skilled Nursing Facility in Millington, TN. The MDS Director will oversee the assessment, care planning, and care delivery processes to ensure high-quality, compliant patient care across the facility.

Ideal candidates hold a Bachelor's Degree in Nursing or Healthcare Administration, 2+ years in a leadership role in skilled nursing, and demonstrate strong MDS knowledge, excellent communication, and

Qualifications

• Bachelor's degree in Nursing, Healthcare Administration, or related field (or equivalent experience).
• Minimum 2+ years in a leadership role, preferably in a Skilled Nursing Facility.
• Strong MDS knowledge with expertise in assessments and care planning.
• Excellent communication with patients, families, and healthcare professionals.
• Strong leadership and management ability to motivate teams.

Responsibilities

• Oversee the entire MDS process, ensuring compliance with federal and state regulations, and accrediting agency standards.
• Develop and implement policies, procedures, and protocols to ensure accurate and timely completion of MDS assessments, care plans, and care delivery.
• Conduct thorough assessments of patients' physical, psychological, and social needs, identifying opportunities for improvement in care delivery.
• Collaborate with the interdisciplinary team to develop individualized care plans that meet patients' specific needs and goals.
• Ensure patients receive high-quality, person-centered care that addresses their physical, emotional, and social needs.
• Directly supervise and mentor MDS coordinators, therapists, and other healthcare professionals to ensure exceptional patient care.
