Job summary

The Brentwood Rehabilitation and Healthcare Center, a 159-bed Sub-Acute and Long-Term Care facility, is seeking an MDS Coordinator. You will oversee timely completion of MDS assessments, ensure regulatory compliance, and collaborate with the DON and care teams to support resident care planning and reimbursement.

Applicants must be a graduate of an accredited nursing program, hold an active RN license, and have at least three years of LTC MDS/RAI experience.

Qualifications

• RN licensed with active, unrestricted status.
• At least 3 years of clinical MDS/RAI in a long-term care setting.
• Strong knowledge of Medicare/Medicaid regulations and reimbursement.
• Excellent accuracy and attention to detail in documentation.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments per regulations.
• Verify compliance with federal/state regulations for MDS submissions.
• Supervise MDS data entry, validation, and transmission.
• Identify and resolve data discrepancies in MDS records.
• Prepare and present MDS Reports to DON and leadership.
• Collaborate with interdisciplinary teams on resident care planning.
• Assist with audit responses and regulatory reviews as needed.
