Job summary

Sequoia Vista at Kaweah Manor seeks an experienced MDS/RAI Coordinator to manage timely MDS coding and care planning for residents in a skilled nursing facility. The role works with the interdisciplinary team to plan assessments, ensure regulatory compliance, and maintain accurate data entry in MDS programs.

The ideal candidate is licensed as an RN or LVN in California with 2+ years in a SNF and a strong understanding of Medicare PPS, Medicaid reimbursement, and OBRA regulations.

Qualifications

• Graduate of an approved RN/LVN program and licensed in California.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, and MDS processes.
• Understanding of care planning and MDS 3.0 coding practices.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Ensure decisions on MDS and assessments meet clinical criteria and regulatory requirements.
• Coordinate daily stand up meetings to review resident care and assessment references.
• Comply with federal and state regulations for RAI process management.
• Monitor MDS and care plan documentation to support coding in medical records.
