Job summary

Sequoia Vista at Kaweah Manor in Selma, CA is seeking an RN/LVN licensed nurse to manage the MDS/RAI processes from admission through discharge. You will collaborate with the interdisciplinary team, coordinate MDS assessments, and ensure regulatory compliance and accurate data entry in MDS systems.

The role emphasizes care planning, Medicare/Medicaid reimbursement understanding, OBRA compliance, and maintaining current MDS status with precise coding from medical records and staff input.

Qualifications

• Graduate of an approved RN / LVN program and licensed in the state of practice, required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of OBRA regulations and Minimum Data Set.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions for MDS assessments based on clinical criteria per the RAI User’s Manual.
• Assist with coordination and management of daily stand up meetings and assessment reference dates.
• Comply with federal and state regulations regarding completion and coordination of the RAI process.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status and accurate data entry in computerized MDS programs.
• Complete accurate MDS coding from medical record review, observations, and interviews.
• Other duties may be assigned at any time with or without notice.
