Job summary

Delmar Gardens is seeking a full-time MDS Registered Nurse (RN) with a Missouri license and MDS experience in long-term care. You will lead assessments, documentation and coordination to support resident care, while ensuring Medicare PDPM reimbursement and Medicaid case-mix accuracy.

The role requires proficiency in EHR systems (MatrixCare a plus) and strong collaboration with an inter-disciplinary team, plus participation in quality improvement initiatives and audits.

Qualifications

• Missouri RN license required for practice in long-term care.
• MDS experience in a long-term care setting is required.
• Proficiency with electronic health record systems, MatrixCare a plus.
• Ability to collaborate with an inter-disciplinary team and participate in QA.

Responsibilities
