The ideal candidate holds an active RN license with MDS experience in a skilled nursing facility, strong knowledge of MDS 3.0, RAI guidelines, PDPM, and Medicare, and excellent organizational and communication skills to collaborate with the interdisciplinary team.

Responsibilities

• Coordinate and complete MDS assessments accurately and timely
• Manage the MDS/RAI process and assessment schedule
• Ensure compliance with CMS, Medicare, Medicaid, and regulatory requirements
• Review clinical documentation for accuracy and completeness
• Work closely with nursing, therapy, social services, and the interdisciplinary team
• Monitor PDPM documentation and reimbursement
• Identify documentation opportunities and ensure appropriate follow-up
• Participate in care planning and interdisciplinary meetings
