Job summary

Delmar Gardens is seeking a full-time MDS Registered Nurse (RN) with a Missouri license and MDS experience in long-term care. You will lead assessments, documentation and coordination to support resident care, while ensuring Medicare PDPM reimbursement and Medicaid case-mix accuracy.

The role requires proficiency in EHR systems (MatrixCare a plus) and strong collaboration with an inter-disciplinary team, plus participation in quality improvement initiatives and audits.

Qualifications

• Missouri RN license required for practice in long-term care.
• MDS experience in a long-term care setting is required.

Job description

We are currently seeking Full-Time MDS Registered Nurses (RN) who possess excellent organizational, leadership, assessment and documentation skills. MDS experience in a long-term care setting and a Registered Nursing license in the state of Missouri is required.

This RN specialist must also be proficient in electronic health record systems; familiarity with MatrixCare EHR software is a plus. In addition, the candidate must have a strong understanding of Missouri Case Mix, participate in quality improvement initiatives and audits and have the ability to work closely with the inter-disciplinary team to ensure the level of care provided is fully supported in the documentation. This nurse plays a key role in obtaining accurate reimbursement for Medicare under the Prospective Payment System (PDPM) as well as Medicaid reimbursement through the Case Mix System.
