Fusion Healthcare Rehabilitation and Nursing Center in New Hampshire is seeking an experienced RN with MDS experience to join our team. You will complete accurate assessments, initiate and update care plans, and help create monthly care plan calendars.

Requirements: NH RN license, 2 years MDS and long-term care experience, strong computer skills, excellent communication, and a team player attitude. We offer health, dental, vision, retirement, paid time off, and a flexible schedule.

Qualifications

• NH RN license is required.
• MDS experience is required.
• Long-term care experience is required.
• Must be highly organized, professional and motivated.

• Experience with MDS tracking and care plans.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.
