Job summary

Warde Senior Living in Windham, NH, seeks an experienced RN to serve as a Full-Time MDS Coordinator within the Nursing Administration Team to ensure compliance, optimize reimbursement, and promote person-centered care.

Lead the RAI/MDS process, maintain schedules, and provide ongoing education on MDS and care planning. The role requires an active NH RN license and 2+ years in long-term care, with on-call rotation. Competitive benefits offered.

Qualifications

  • RN with NH license and 2+ years in long-term care, including RAI experience.
  • Experience with the RAI/MDS process and person-centered care planning.
  • Strong clinical judgment and communication abilities.

Responsibilities

  • Lead the RAI/MDS process including care planning and PDPM reimbursement.
  • Maintain Medicare/Medicaid assessment schedules and distribute to teams.
  • Provide ongoing education on MDS and care planning.
  • Oversee RAI processes, triggers, CAAS and care planning.
  • Maintain Medicare certification binder to ensure compliance.
  • Stay current on long-term care regulations and reimbursement rules.
  • Support audits and template/tool development.
  • Utilize MS Office for reporting and training.

We are seeking an experiencedRN to serve as our Full-Time MDS Coordinator, joining the Nursing Administration Team to ensure compliance, optimize reimbursement, and promote person-centered care.

  • Participation in the on-call rotation as part of the facility leadership team is required.

Key Responsibilities:
  • Lead the RAI/MDS process including person-centered care planning, PDPM reimbursement, Medicare certification, scheduling, month-end triple check, and appeals
  • Maintain PDPM/Medicare and Medicaid assessment schedules and distribute to therapies, dietary, social service, activities, and nursing teams
  • Provide ongoing education and training on MDS and care planning
  • Oversee the RAI process, including MDS assessments, triggers, CAAS, and care planning
  • Maintain Medicare certification/recertification binder to ensure compliance with federal Part A & B services
  • Stay current on long-term care regulations, best practices, and reimbursement requirements
  • Support the facility in audit and educational tools, including template development
  • Utilize Microsoft Office (PowerPoint, Word, Excel, Outlook) for reporting, tracking, and training
Requirements
  • Registered Nurse (RN) with an active New Hampshire license
  • Minimum of 2 years’ experience in long-term care, including experience with the RAI process and person-centered care planning
  • Strong clinical judgment and excellent communication skills
  • In-depth knowledge of the RAI process and federal and state reimbursement requirements
  • Highly organized with the ability to manage multiple priorities throughout the day
  • Proficient in Microsoft Word, Excel, PowerPoint, and Outlook
  • Experience creating templates and tools for audits, tracking, and staff education
  • Participation in the on-call rotation as part of the facility leadership team is required
