Job summary

Fletcher Rehabilitation and Healthcare Center in Fletcher, NC is hiring a full-time MDS Coordinator to manage MDS processes and care plans for Residents in a long-term care setting. The role requires nursing licensure, CPR certification, and knowledge of MDS procedures, with preference for 1 year LTC experience.

Qualifications

• Current State License as a Nurse.
• CPR certification.
• Knowledge and training in all aspects of the MDS process.
• One year experience as a Nurse in a long-term care setting (preferred).

Responsibilities

• Develop comprehensive care plans for Residents in coordination with the MDS that address Resident needs and goals.
• Coordinate, manage, and monitor the written plan of care for each Resident.
• Review nurse's notes to ensure the care plan is being followed.
• Monitor Resident status changes and coordinate timely nursing or clinical involvement.
• Ensure MDS and supporting documents accurately reflect the Resident and meet regulatory/auditor requirements.
• Perform regular audits of documentation for accuracy.
• Assist with discharge processes and complete necessary forms/reports.
• Maintain confidentiality of Resident health information and uphold safety precautions.
