Job summary

McLaren Health Care in Lake Orion is seeking a Registered Nurse to join the MDS team at the Lake Orion Nursing & Rehab Center. You will coordinate care across disciplines, complete MDS assessments using current guidelines, and support the Medicare Case Manager and nursing staff.

The role emphasizes collaboration, education, discharge planning, and ensuring accuracy and compliance with federal and state regulations.

Qualifications

• Graduate from an accredited school of nursing.
• Must be currently licensed to practice as a Registered Nurse and hold a valid Michigan State License.
• Be MDS 3.0 certified (or equivalent).
• Demonstrate competence in assessment skills and computer literacy.
• Possess good organizational and communication skills.

Responsibilities

• Coordinate care for residents using a holistic approach.
• Collaborate with interdisciplinary teams and liaise with families, physicians and home care as needed.
• Maintain comprehensive knowledge of residents’ medical condition and educational needs.
• Support floor staff with education and best practices in care delivery.
• Participate in discharge planning and coordinate discharge notices.
• Attend resident care plan meetings as nursing representative.
• Communicate daily with nurse manager and rehab director for care coordination.
