Regency at Whitmore Lake is seeking an experienced Care Management Nurse (MDS Nurse) to handle the RAI process, assessments, and care plan coordination for residents. The role requires completing MDS, CAA, and care plans within regulated timeframes, and coordinating with physicians, nursing, social services, and therapy staff.

Formación

• Active nursing license (RN or LPN).
• Knowledge of the RAI process; PPS strongly preferred.
• Experience as an MDS nurse.

Responsabilidades

• Performs MDS duties, CAAs and care plans within required timeframes.
• Assesses residents via physical exam, interview and chart review.
• Collaborates with physicians, nursing, social services, therapy, dietary and activity staff.
• Reviews hospital and outside information to inform assessments and planning.
• Maintains PPS board and monitors Medicare days and RUGs when needed.
