Ciena Healthcare's Royalton Manor in Michigan is seeking an experienced registered nurse to stay clinically involved in patient care in a non-bedside role. The Care Management Nurse, MDS Nurse will manage the RAI process, coordinate care plans, and work with a multidisciplinary team.

Qualifications

• Active RN license in Michigan.
• Knowledge of the Resident Assessment Instrument (RAI) and Prospective Payment System (PPS) preferred.
• Experience as an MDS Nurse.

Responsibilities

• Completes the MDS, CAAs and care plans within regulated time frames.
• Assesses residents via physical assessment, interview and chart review.
• Coordinates discussions with physicians, nursing, social services, therapy, dietary and activity staff.
