Optalis Health & Rehabilitation is seeking an experienced MDS Coordinator to manage the MDS/RAI process, ensuring accuracy, timely submission, and CMS-compliant documentation. You will support Medicare/Medicaid reimbursement and promote high-quality resident care.

The role requires an RN or LPN license in Michigan with prior MDS coordination experience, strong analytical skills, and proficiency with EHR/MDS software. Collaborates with nursing and billing to optimize outcomes.

• MDS/RAI process knowledge and CMS reimbursement guidelines.
• RN or LPN license in Michigan without restrictions.
• Experience in MDS coordination or similar nursing leadership role.
