Job summary

Friendship Health and Rehab in New Jersey is seeking a PRN MDS Coordinator to uphold RAI guidelines and conduct MDS assessments and care planning for residents. The MDSC will focus on optimizing PDPM and RUGs 48 reimbursement, while assisting with Managed Care updates as needed.

The role requires RN or LPN with MDS 3.0 knowledge and a nursing degree, with experience in long-term care MDS. Join a team dedicated to Resident Centered Care and regulatory readiness.

Qualifications

• Nursing degree from an accredited institution
• RN or LPN with MDS 3.0 knowledge – certification preferred
• Experience in Long-term Care MDS

Responsibilities

• Maintain MDS guidelines and timely care planning
• Collaborate with Interdisciplinary team to develop Resident Centered Care Plans
• Provide Managed Care updates to Case Managers
• Ensure readiness for regulatory inspections
• Coordinate with physicians and offices as needed
