Job summary

Tandem Health Care is seeking an MDS Nurse-RN to coordinate services for managed care and Medicare residents in collaboration with the facility team. The role supports the Director of Clinical Services by ensuring documentation meets federal and state requirements and overseeing the MDS, RAPS, and Interdisciplinary Care Plan processes.

Qualifications

• MDS Nurse-RN responsible for coordinating services for managed care and Medicare residents.
• Ability to document in accordance with federal and state guidelines.
• Work with interdisciplinary team to ensure high quality resident care.

Responsibilities

• Conduct and document a thorough assessment of each resident's medical status upon admission and throughout the resident's course of treatment.
• Under the supervision of the Registered Nurse, assist in the implementation of the resident's treatment plan.
• Provide routine nursing services for residents as directed by Registered Nurse/Nursing Supervisor.
• Comply with evaluation, treatment and documentation guidelines of Tandem Health Care.
• Complete required documentation in an accurate and timely manner.
• Provide regular resident status updates to appropriate personnel.
• Attend and participate in department/facility meetings, as required.
• Act in compliance with Tandem, regulatory and professional standards and guidelines.
• Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency.
• Adheres to facility policies and procedures and participates in facility quality improvement and safety programs.
