Job summary

North Mississippi Health Services is seeking an MDS Coordinator to coordinate patient care and optimize resource use within Case Management. The role emphasizes discharge planning, adherence to care criteria, and collaboration with the care team to support timely and appropriate patient progression.

You will monitor patient status, participate in planning, ensure regulatory compliance, and work with physicians, nurses, and payers to ensure accurate documentation and billing for continued care

Qualifications

• Experience with MDS and care planning processes.
• Ability to read and interpret medical record documentation.
• Strong knowledge of regulatory and accrediting standards.
• Excellent communication and coordination skills.

Responsibilities

• Monitors cases relative to criteria, LOS and guidelines.
• Ensures level of care criteria are met; issues denials when needed.
• Intervenes to promote timely care and resource use.
• Serves as resource for physicians and nurses on medical necessity.
• Monitors outcomes to evaluate clinical and financial effectiveness.
• Reconciles services with patient benefits; communicates with physicians and payers.
• Manages social work and case management to support discharge planning.
• Collaborates with care team to plan continued care across levels.
• Assesses social/emotional factors and makes referrals.
• Participates in discharge planning early in treatment.
