Job summary

Memorial City Nursing and Rehabiliation Center seeks a Care Management Specialist to coordinate the RAI process, ensuring timely MDS completion and accurate resident status documentation. You will collaborate with the interdisciplinary team to optimize care plans and reimbursement.

Responsibilities include pre-admission ARD data collection, MDS accuracy audits, scheduling, and ongoing communication with Case Management, Business Office, and external partners.

Qualifications

• Must be a graduate of an accredited school of nursing with current RN or LVN licensure.
• Experience with MDS/RAI, reimbursement, and case management preferred.
• Strong documentation and analytical skills; ability to lead interdisciplinary teams.
• Knowledge of CMS RAI guidelines and federal/state regulations.

Responsibilities

• Coordinate the RAI process for timely and accurate MDS completion.
• Collaborate with the interdisciplinary team to develop and manage care plans.
• Maintain accurate MDS reference dates (ARD) and compliance with CMS rules.
• Audit coding data, validate outcomes, and ensure data integrity reports.
• Communicate with Regional Care Management Specialist and Business Office.
