Responsibilities include preparing RAI, coordinating schedules, and maintaining MDS software, while collaborating with nursing, therapy, and clinical staff to optimize care plans and documentation.

Qualifications

• Experience with RAI/MDS processes in long-term care facilities.
• Strong communication across care teams and disciplines.
• Ability to review medical documentation for accuracy and compliance.

Responsibilities

• Prepare and manage RAI/MDS data and schedules.
• Coordinate completion of RAI per state and federal guidelines.
• Notify team with assessment dates and distribute schedules.
• Assist with RAI completion and data input.
• Adjust assessment schedules to optimize RUG scores.
• Screen MDS for accuracy before finalizing and signing.
• Support Medicare review and ensure appropriate coverage.
