Job summary

Qualifications

• Active LPN/LVN license in good standing in the applicable state (Massachusetts).
• Minimum of 2 years nursing experience; geriatric experience preferred.
• CPR certification at hire or within orientation and maintain current.

Responsibilities

• Complete and submit accurate MDS Assessments, CCAs, and Care Plans per CMS RAI Manual.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI updates affecting documentation.
• Review Interdisciplinary Care Plan and CMS Reports; ensure timely billing readiness.
