Job summary

Life Care Centers of America seeks an LPN MDS Nurse to complete and submit accurate MDS assessments, CCAs, and care plans in accordance with CMS RAI Manual regulations and Life Care standards.

The role requires an active LPN license, 2 years of nursing experience (geriatric preferred), and CPR certification trained by hire or during orientation. Strong understanding of post-acute care regulations is essential.

Qualifications

• Attended an accredited LPN program.
• Active LPN license in good standing.
• Two years of nursing experience; geriatric experience preferred.
• CPR certification obtained by hire or during orientation.

Responsibilities

• Complete and submit accurate MDS Assessments, CCAs, and Care Plans per CMS RAI Manual.
• Report patient condition changes identified by MDS to the DON.
• Educate direct care staff on CMS RAI Manual updates affecting documentation.
• Assist with Interdisciplinary Comprehensive Care Plan review.
• Review CMS Reports and ensure assessments are billed or flagged for correction.
