Job summary

PACS Group is seeking an MDS Nurse (LVN/LPN) to coordinate MDS assessments, develop care plans, and support reimbursement accuracy. You will ensure regulatory compliance and work with an interdisciplinary team to optimize resident outcomes.

The role requires an active LVN/LPN license in California, with experience in long-term care and proficiency in MDS 3.0, RAI, and EHR systems. Strong organizational and communication skills are essential.

Qualifications

• Knowledge of MDS 3.0, RAI, and Medicare/Medicaid reimbursement guidelines.
• Experience in long-term care or skilled nursing facilities preferred.
• Strong organizational skills and attention to detail.
• Ability to work with interdisciplinary teams and maintain regulatory compliance.

Responsibilities

• Complete MDS assessments and CAAs accurately within regulatory timelines.
• Ensure documentation supports individualized care planning and reimbursement.
• Maintain CMS/state regulatory compliance and facility policies.
• Collaborate with nursing, therapy, social services, and other teams to update care plans.
