Marin Post Acute in San Rafael, CA is seeking an LVN MDS Coordinator to oversee the MDS/RAI process and care-planning for residents. This full-time role offers $43-$50/hour DOE with comprehensive benefits.

The coordinator will ensure CMS/state compliance, collaborate with an interdisciplinary team, and monitor PDPM documentation and reimbursement accuracy. California LVN license and 1+ year MDS experience are required.

Qualifications

• Active California LVN license.
• Minimum 1 year MDS experience in a SNF or post-acute setting.
• Knowledge of MDS 3.0, RAI process, PDPM, and Medicare/Medicaid reimbursement.
• Strong assessment, documentation, and organizational skills.
• Ability to work independently while collaborating with teams.

• Coordinate and manage the MDS/RAI process in accordance with CMS, federal, and state regulations.
• Complete accurate and timely MDS assessments and supporting documentation.
• Develop and update comprehensive resident care plans.
• Collaborate with nursing, therapy, dietary, and interdisciplinary team members.
• Monitor PDPM documentation and reimbursement accuracy.
• Track quality measures and identify opportunities for clinical improvement.
• Assist with survey preparation, audits, and regulatory compliance initiatives.
• Educate and support staff regarding MDS processes and documentation requirements.
• Partner with facility leadership to ensure clinical and operational excellence.

Qualifications Required

• Current, active California LVN license
• Minimum 1 year of MDS experience in a Skilled Nursing Facility (SNF) or Post-Acute setting
• Knowledge of MDS 3.0, RAI process, PDPM, Medicare, and Medicaid reimbursement
• Strong assessment, documentation, and organizational skills
• Ability to work independently while collaborating with interdisciplinary teams

Preferred

• RAC-CT certification
• Experience with PointClickCare
• Prior skilled nursing facility leadership experience
