Job summary

Victorian Post Acute in San Francisco, CA is seeking an MDS Nurse (LVN/LPN) to coordinate MDS assessments, develop individualized care plans, and ensure regulatory compliance.

You will work with an interdisciplinary team to support reimbursement accuracy and maintain high-quality resident care, while staying current with CMS and state requirements.

Active LVN/LPN license in California and prior experience in long-term care are preferred; strong documentation and communication skills are essential.

Qualifications

• Current, active LVN/LPN license in the state of practice.
• Experience in long-term care or skilled nursing facility preferred.
• Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines.
• Strong organizational skills and attention to detail.
• Ability to work effectively with interdisciplinary teams.
• Proficiency in electronic health records (EHR) and Microsoft Office Suite.
• Strong written and verbal communication skills.
• Must maintain licenses and CE in good standing.

Responsibilities

• Complete MDS assessments and CAAs accurately within regulatory timelines.
• Ensure clinical documentation supports care planning and reimbursement.
• Maintain CMS/state regulatory compliance and facility policies.
• Collaborate with nursing, therapy, social services, dietary, and other teams to develop/update care plans.
• Monitor progress and ensure assessments reflect current needs.
• Participate in care conferences, audits, surveys, and QI initiatives.
• Ensure timely MDS submissions for Medicare/Medicaid reimbursement.
• Stay informed of regulatory updates affecting MDS/documentation.
• Educate staff on documentation standards and MDS requirements.
• Communicate with residents and families regarding care planning.
