PACS is seeking an MDS Nurse (LVN/LPN) to coordinate and complete MDS assessments in compliance with federal and state regulations. You will ensure accurate documentation, support reimbursement, and collaborate with interdisciplinary teams to promote quality outcomes for residents.

Ideal candidates hold an LVN/LPN license, with preferred experience in long-term care, knowledge of MDS 3.0/RAI, and proficiency in EHR systems.

Qualifications

• Current LVN/LPN license in the state of practice.
• Experience in long-term care or skilled nursing facility preferred.
• Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines.
• Strong organizational skills with attention to detail.
• Ability to work collaboratively with interdisciplinary teams.
• Proficiency in electronic health records (EHR) systems.

Responsabilités

• Coordinate and complete MDS assessments within required timelines.
• Ensure accuracy of resident documentation to support care planning and reimbursement.
• Maintain CMS/state/facility compliance.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor resident progress and update care plans as needed.
• Participate in care conferences and quality improvement initiatives.
• Support MDS submissions for Medicare/Medicaid reimbursement.
• Assist with audits and surveys with accurate documentation.
• Stay current with regulatory changes affecting MDS.
