Job summary

Arlington Pointe Care Center is seeking an experienced MDS Nurse - LPN to lead resident assessments, care plans, and quality improvement activities. The role requires coordinating with the interdisciplinary team, discharge planning, and ensuring regulatory compliance.

The incumbent will administer and update MDS processes, participate in facility surveys, and support staff through training and supervision to maintain high-quality resident care.

Qualifications

• Must possess a current, unencumbered LPN license in the relevant state.
• Ability to read, write, speak, and understand English.
• Demonstrates leadership, teamwork, and the willingness to mentor others.

Responsibilities

• Coordinate and develop resident assessments (MDS) per current rules and RAPs/Triggers.
• Maintain policies and procedures for MDS and care plans.
• Perform administrative duties and prepare medical forms and reports.
• Develop and maintain an ongoing quality assurance program for MDS and care plans.
• Assist in discharge planning and coordinate with care plan team.
