Job summary

Americare Senior Living is seeking a detail-oriented MDS Coordinator (LPN) to support accurate assessments and compliant documentation within a skilled nursing community. You will coordinate MDS processes, support resident care, and collaborate with interdisciplinary teams to ensure high-quality outcomes.

The role emphasizes regulatory adherence, survey readiness, and reimbursement accuracy, contributing to a safe, home-like environment for residents and a strong focus on resident-centered care.

Qualifications

• 3–5 years healthcare experience preferred
• 1+ year clinical experience in a Skilled Nursing Facility preferred
• 2+ years long-term care or assisted living experience preferred
• Knowledge of Medicare reimbursement, RUGs, and state/federal regulations

Responsibilities

• Coordinate and complete MDS assessments per federal and state guidelines
• Maintain admission nursing notes, plan of care, and required documentation per standards
• Participate in Medicare meetings, care plan meetings, and IDT discussions
• Assist Director of Nursing with compliance and survey readiness
• Communicate significant changes in resident condition to leadership
• Assist with resident admissions, placements, and room transfers with IDT coordination
• Schedule and document nursing staff meetings and review incident reports
