Jade Financial is seeking an MDS nurse (Minimum Data Set) to conduct comprehensive resident assessments in a long-term care setting. You will collect data using standardized forms and help shape personalized care plans in collaboration with the care team.

The role requires an active Illinois RN/LPN license, strong assessment and communication skills, and a solid understanding of federal regulations governing long-term care and Medicare/Medicaid reimbursement.

Qualifications

Responsibilities

• Conduct resident assessments using the MDS tool and collect comprehensive health data.
• Document and verify data accurately on the MDS form for care planning.
• Develop individualized care plans based on MDS findings and resident needs.
• Collaborate with physical therapists, occupational therapists, dietitians, and other team members to implement care plans.
• Monitor compliance by ensuring timely and accurate MDS submissions to meet regulatory requirements.
• Communicate resident needs to families, team members, and facility management.
• Participate in quality improvement initiatives to enhance resident care and compliance.
