Job summary

Jade Financial is seeking an experienced MDS Nurse/Care Plan Coordinator (LPN/RN) to ensure accurate assessments and up-to-date care plans for residents, and to maintain EMR documentation aligned with the plan of care. You will communicate with families and coordinate Comprehensive Resident Assessments under the direction of the Administrator.

Responsibilities include coordinating MDS completion, updating care plans, training staff, reviewing diagnosis coding with physicians, and ensuring care

Qualifications

• Current State of Illinois license as LPN or RN.
• Read, write, and speak English clearly.
• AANAC certification preferred.

Responsibilities

• Coordinate the completion of the MDS at admissions, readmissions, quarterly reviews, annually, and when significant changes occur.
• Monitor EMR charting and ensure resident care plan goals are met.
• Provide in-service training on care plans and documentation.
• Review diagnosis coding with physicians and update ICD-9 coding as needed.
• Communicate care plans and progress to staff, residents, and families.
