Job summary

Epic Healthcare in California is seeking an experienced nurse with expertise in MDS assessments and care planning to ensure residents receive quality, individualized care.

You will complete and coordinate MDS assessments, develop and maintain care plans, and collaborate with the interdisciplinary team to ensure timely documentation and regulatory compliance.

Qualifications

• Current RN or LVN/LPN license in California, as applicable.
• MDS experience preferred.
• Knowledge of MDS/RAI requirements and long-term care regulations.
• Strong organizational and communication skills.
• Ability to work independently and meet assessment deadlines.
