Job summary

LongTerm Healthcare is seeking an organized MDS Nurse / MDS Coordinator to manage the MDS/RAI process and care planning. You will work closely with nursing, therapy, dietary, social services, and physicians to translate resident data into accurate, person-centered plans.

The ideal candidate has strong knowledge of MDS 3.0, RAI processes, and regulatory requirements, with attention to detail and excellent deadlines management.

Qualifications

• Knowledge of MDS 3.0 and RAI processes required.
• Strong clinical documentation and care planning skills.
• Experience coordinating interdisciplinary teams in SNF/long-term care.

Responsibilities

• Coordinate the MDS/RAI process and ensure timely completion of MDS assessments.
• Review clinical documentation for accuracy and consistency.
• Lead care planning to reflect resident needs and preferences.
• Collaborate with nursing, therapy, dietary, social services, and physicians.
• Participate in care plan and interdisciplinary team meetings.
• Ensure regulatory compliance and survey readiness.
