Job summary

SNF Staffing Solutions is seeking an MDS Coordinator to oversee timely completion of the RAI process and care management from admission to discharge in line with federal, state, and facility guidelines.

The role requires an RN license, at least 2 years in a Skilled Nursing Facility, and strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement. MDS 3.0 experience is preferred.

Qualifications

• RN license in good standing in state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and MDS requirements.
• MDS 3.0 experience preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Determine whether to complete additional MDS assessments based on clinical criteria.
• Manage daily stand-up meetings and set assessment reference dates.
• Ensure compliance with federal and state MDS regulations.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per guidelines and update as needed.
• Enter data into computerized MDS programs accurately and regularly.
• Code MDS information from records, observations, and interviews.
• Attend interdisciplinary and quality assurance meetings to share updates.
