Job summary

RAM Recruiting - The Pavilion at Edgefield in Canton, Ohio is seeking an experienced MDS Coordinator to join our clinical leadership team. This role focuses on ensuring accurate MDS assessments, RAI processes, and PDPM-based reimbursement across a full-continuum skilled nursing campus.

The ideal candidate holds an active RN or LPN license in Ohio, has prior MDS/LTC experience, and can coordinate with nursing, therapy, and the interdisciplinary team to maintain survey readiness and timely

Qualifications

• Active RN or LPN license in Ohio.
• Prior MDS experience in a SNF/LTC setting.
• Familiar with RAI, PDPM and timely assessments.

Responsibilities

• Coordinate and complete MDS assessments accurately and on schedule.
• Manage the RAI process including care plan coordination.
• Ensure documentation supports accurate PDPM-based reimbursement.
• Collaborate with nursing, therapy and interdisciplinary team on assessments and care planning.
• Monitor assessment schedules and transmission deadlines to stay survey-ready.
